The monument commemorates the crash of the Royal Australian Air Force Catalina flying boat, A24-381.

Flight Lieutenant Smith was the captain of Catalina A24-381 which crashed at Lord Howe Island on the evening of 28 September 1948, taking the lives of seven of her nine crew members, including that of Flight Lieutenant Smith.
