The plaque commemorates the site of the crash of the Royal Australian Air Force Catalina flying boat, A24-381 in 1948.
Flight Lieutenant Smith was the captain of Catalina A24-381 which crashed at Lord Howe Island on the evening of 28 September 1948, taking the lives of seven of her nine crew members, including that of Flight Lieutenant Smith.

On 28th September, 1948 a R.A.A.F Catalina Flying Boat A24-381 from No 11 Squadron crashed on this spot after dark.
Seven of the crew of nine men were killed.
