The plaque commemorates all those who served in H.M.A.S. Cape Leeuwin, Survey and Lighthouse Support from 1943 to 1945.

It is also dedicated to requisitioned ships Alert, Bangalow, Kwato, Polaris, Stella, Winter and other support ships of World War Two.
