The plaques commemorate the Ballan and District Soldiers Memorial Bush Nursing Hospital and Hostel which was opened in 1966 and dedicated to all servicemen who died in service or were killed in action in the various conflicts in which Australia has been involved.
The original Memorial Bush Nursing Hospital has been demolished and replaced with a new hospital building. The plaques from the original building were found in a storage shed at the hospital.
