The monument over the grave commemorates William Forsythe, past Provincial Grand Master of the Provincial Grand Black Chapter of New South Wales, who died in 1900. 

Yesterday a large number of number of the Orange Lodge and Provincial Grand Black Chapter met at the Rookwood Cemetery for the purpose of unveiling a monument erected in memory of the late William Forsythe, P.P.G.M. The attendance included Bro. Oliver Paul (Provincial Grand Master), Sir Knights Ferry, Chandler, Chaplin, Lucas, Lloyd, Stokes, Wiggins,  Bros.E.Morman, T Manuel, (Grand Treasurer), E Elliott, King, A.P. Elliott, H Robey, Lennox, A Bennett (W.M.), J Sheridan, H. Davis, H. J. Nicholls, J Branken, and T Henderson, as well as relatives. Bro. Paul unveiled the memorial and delivered an address. He also read a letter from the Grand Chaplain, the Rev T. J. Curtis, in which kindly reference to the good qualities of the deceased was made. Addresses were also delivered by Bros Perry, Morman, and Chandler.  The enclosure was over two allotments.

A Pyrmont stone base surmounted by bluestone and an iron railing surrounded the grave, which was covered with marble chips on concrete. The monument itself was placed on a bluestone and marble base, and was of marble. At the top was a draped marble urn. The whole monument was of a well finished and rich character, and was made and placed in position by F Arnold, of Regent-street. The inscription was as follows -"In loving memory of William Forsythe, P.P.G.M., who died June 12, 1900, aged 45. 'Let there be light ' Erected by the Provincial Grand Black Chapter of New South Wales." The hymn " God be with You" was sung and the ceremony concluded.
