The memorial plaque commemorates the Royal Australian Navy (R.A.N.) Sloops H.M.A.S. Yarra, H.M.A.S. Swan, H.M.A.S. Parramatta and H.M.A.S. Warrego and the personnel who lost their lives or were wounded while serving on them.
