Plaque commemorate the Ballan and District Soldiers Memorial Bush Nursing Hospital and Hostel which was opened in 1966 and dedicated to all servicemen who paid the supreme sacrifice in times of conflict.
The original Memorial Bush Nursing Hospital has been demolished and replaced with a new hospital building.
Note : Plaques from the original building were found in a storage shed at the hospital.
